They still have the old form there also, so be careful. Also, the expiration date is 2022 not 2020.
Are there any real changes that we all need to investigate?
@holy-cow I took a quick look at it and didn’t see any changes other than the expiration date. I did not try to fill it with data so there could be some surprises there.
I made a typo on the expiration date, 11-30-2022 is the correct expiration date
regarding this version….
I did not like the method the pdf places your stamp and
I did not like the way it does not adopt the file name as the print to pdf file name
I’ve modified this using adobe livecycle to remedy this
my revised format is attached
it also would not print to pdf using the my acrobat driver so I used the microsoft print to pdf driver and it printed fine
I have not looked at the instructions to see if there are any changes
I have been placing mention in the notes that the certificate is not valid unless also signed and stamped on the first page. No one has had an issue with that.
Thanks Stephen, I have learned, the hard way, to listen to your Elev Cert Form advice. I have two to do next week. I hope they go better than the last time FEMA updated their form.

I’ll have to take a closer look at it and see if it can be tweaked without starting from scratch.
